Terms of Service

1. Acceptance of Terms
By creating an account, accessing, or using forbiddenrx.co (the “Service”), you agree to be bound by these Terms of Service (“Terms”), our Privacy Policy, and our Telehealth Informed Consent. If you do not agree, do not use the Service.
2. Who We Are
ForbiddenRx is a brand operated by ForbiddenRX LLC, a Delaware limited liability company (the “Platform”). The Platform is a technology and administrative services provider that connects patients with independent licensed medical providers.
Medical care is delivered by Jimenez Medical Wellness, PLLCand other affiliated medical practices (the “Medical Group”). The Platform does not practice medicine, employ providers in their clinical capacity, or interfere with medical decisions. Each provider is independently responsible for their clinical judgment.
3. Eligibility
- You must be at least 18 years of age
- You must reside in a U.S. state where our providers are licensed at the time of your visit
- You must provide accurate, current, and complete information
- You must not be subject to a court order or law that prohibits your use of telehealth services
4. The Service Is Not a Pharmacy or Medical Practice
The Platform is not a pharmacy, does not stock or dispense medications, and does not employ pharmacists. Approved prescriptions are routed to and filled by independent licensed United States pharmacies. The Platform is not a medical practice and does not provide medical advice; it provides the technology and administrative infrastructure that enables licensed providers to evaluate and treat you.
5. No Guarantee of Prescription
Payment of any fee does not guarantee a prescription.The treating provider has sole clinical discretion to determine whether a prescription is medically appropriate for you. The provider may approve, modify, deny, or recommend an alternative course of treatment. Denial of a prescription is not a refundable event — the consultation fee compensates the provider for the time and clinical judgment used to evaluate you.
6. Pricing
Consultation Fee
A $35 consultation fee is charged at the time you book your initial telehealth visit. This fee compensates the provider for clinical evaluation regardless of whether a prescription is approved.
- If your provider approves a prescription: the $35 consultation fee is credited toward your first medication purchase, so the medication price you see at checkout is the total you pay.
- If your provider denies a prescription: the $35 consultation fee is non-refundable and no further charges are made.
Medication Subscriptions
Most treatments are sold as monthly subscriptions with a single recurring charge billed on the same day each month. Subscription pricing is shown before checkout and includes the medication, dispensing, and standard shipping. Subscription pricing varies by treatment.
Insurance
ForbiddenRx is a cash-pay service. We do not accept or bill health insurance. You may submit our receipts to your insurance for possible out-of-network reimbursement at your own discretion; reimbursement is not guaranteed.
7. Cancellations and Refunds
- Consultation fee: non-refundable once your visit is conducted, regardless of outcome.
- Subscriptions: you may cancel any active subscription through your patient portal at any time. Cancellations made before the next billing date prevent the next charge. Cancellations made after billing apply to the following month.
- Shipped medications: federal and state law prohibits the return of dispensed prescription medications. Once a prescription has been filled and shipped, it is non-returnable and non-refundable.
- Damaged or lost shipments: contact support@forbiddenrx.co within 7 days of expected delivery and we will work with the dispensing pharmacy on a remedy.
8. Acceptable Use
You agree not to:
- Provide false or misleading information during intake or any visit
- Resell, distribute, or share medications obtained through the Service
- Attempt to access another user’s account or PHI
- Reverse engineer, scrape, or interfere with the Service
- Use the Service for any unlawful purpose
9. Disclaimers
The Service is provided “as is” without warranties of any kind, express or implied. We do not warrant that the Service will be uninterrupted, error-free, or meet your particular needs. The Platform makes no representation or warranty regarding the outcome of any medical evaluation or treatment.
The Service is not for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately.
10. Limitation of Liability
To the fullest extent permitted by law, the Platform’s total liability for any claim arising out of or relating to these Terms or the Service is limited to the amount you paid to the Platform in the twelve months preceding the claim. The Platform is not liable for clinical decisions made by independent providers.
11. Indemnification
You agree to indemnify and hold harmless the Platform and its affiliates from any claim or demand arising out of your breach of these Terms, your misuse of the Service, or your violation of any law.
12. Governing Law and Disputes
These Terms are governed by the laws of the State of Delaware, without regard to its conflict of laws principles. Any dispute arising under these Terms shall be resolved by binding arbitration administered by JAMS in Wilmington, Delaware, except that either party may seek injunctive relief in court for intellectual property or confidentiality matters.
13. Changes to These Terms
We may update these Terms from time to time. Material changes will be communicated by email and posted on this page with a revised “Last updated” date. Continued use of the Service after changes constitutes acceptance.
