Telehealth Informed Consent

1. What Telehealth Is
Telehealth is the delivery of healthcare services using electronic communication technologies — including secure video, audio, messaging, photographs, and other digital tools — instead of an in-person visit. Through ForbiddenRx, you may interact with a licensed medical provider remotely for evaluation, diagnosis, prescribing, and follow-up care.
2. Who Provides Your Care
Care is provided by independent licensed medical providers who hold an active license to practice medicine in your state of residence at the time of your visit. The identity, credentials, and license information of your treating provider will be disclosed to you during the intake and care process. Your provider exercises independent clinical judgment.
3. Benefits of Telehealth

- Reduced travel time, cost, and exposure to other patients
- Faster access to evaluation for many treatable conditions
- Consistent follow-up and refill coordination
4. Risks and Limitations
You understand and accept that telehealth has limitations:
- No physical examination. Your provider cannot perform a hands-on exam, take vital signs in person, or order in-office testing during the visit.
- Information you provide is essential. The accuracy of your medical history, symptoms, and any photos or measurements you submit directly affects the quality of clinical decision-making. Inaccurate or incomplete information may result in incorrect or harmful treatment.
- Technology can fail. Connections may drop, audio or video quality may be poor, and data may be delayed. We use secure systems, but no electronic transmission is 100% private or reliable.
- Some conditions are not appropriate for telehealth. Your provider may decline to treat you and refer you to in-person care if telehealth is not safe or appropriate for your situation.
- Adverse outcomes may occur with any treatment, in person or remote. Your provider will discuss material risks of any prescribed treatment with you.
5. Prescription Approval Is Not Guaranteed
Your provider will determine whether prescription medication is medically appropriate based on your evaluation. Payment of any fee does not entitle you to a prescription. If your provider determines that medication is not appropriate, no prescription will be issued, and the consultation fee remains payable.
6. Electronic Communications
You consent to receive communications from ForbiddenRx and your provider electronically — including email, SMS text messages, in-app messages, and secure portal messages — for appointment reminders, prescription notices, refill alerts, and other treatment-related purposes. Standard message and data rates may apply. You may opt out of non-essential marketing messages at any time, but treatment-related communications are required for safe care.
7. Confidentiality and PHI
Information shared during telehealth visits is treated with the same confidentiality as in-person care under HIPAA. Your protected health information may be shared with the dispensing pharmacy, your treating provider, and other clinicians involved in your care. 8. Recordings
Visits may be recorded for medical record, quality assurance, and compliance purposes. Recordings are retained as part of your medical record. You may not record or share your visit with third parties without the provider’s written consent.
9. Emergencies
The Service is not for medical emergencies. If you are experiencing a life-threatening condition — including chest pain, difficulty breathing, suicidal thoughts, severe bleeding, or signs of stroke — call 911 or go to the nearest emergency department immediately. Do not attempt to use the Service in place of emergency care.
10. Right to Withdraw Consent
You may withdraw your consent to telehealth at any time without affecting your right to future in-person care from any provider. Withdrawing consent will end your active care through the Service. To withdraw consent, email support@forbiddenrx.co.
11. Acknowledgment
By proceeding through intake and submitting a request for evaluation, you acknowledge that you have read this Telehealth Informed Consent, understand the benefits and risks of telehealth, and consent to receive care through the Service. You may review this document at any time at this URL.
