Our Providers

Every prescription written through ForbiddenRx is issued by a licensed medical provider who has individually evaluated you. ForbiddenRx is a technology platform — we do not employ providers, we do not direct clinical decisions, and we do not write prescriptions. Care is delivered by an independent medical group.
1. Who Delivers Your Care
Clinical services on the ForbiddenRx platform are provided by independent licensed medical providers practicing through Jimenez Medical Wellness, PLLC, a professional limited liability company organized under the laws of the State of New York (the “Medical Group”), and through affiliated medical practices contracted to deliver telehealth in additional states.
The Medical Group contracts with licensed physicians, physician assistants, and nurse practitioners who hold an active, unrestricted license in the state where you reside at the time of your visit. Each provider exercises independent clinical judgment in evaluating you, recommending or declining treatment, and writing or declining to write any prescription.
2. Provider Credentialing
Before a provider is permitted to see patients on the ForbiddenRx platform, the Medical Group verifies:
- Active, unrestricted license to practice in each state in which they treat patients
- Completion of an accredited training program for their discipline (MD, DO, PA, NP)
- Current board certification or eligibility, where applicable
- No active license suspensions, revocations, or pending disciplinary actions
- National Practitioner Data Bank query, where required
- Professional liability (malpractice) coverage in force
- Identity verification and a satisfactory background check
Credentials are reverified on an ongoing basis and at the cadence required by applicable state law and accreditation standards.
3. Clinical Oversight
The Medical Group is led by a Medical Director who is a physician licensed in good standing in the State of New York. The Medical Director is responsible for:
- Approving the clinical protocols used on the platform
- Overseeing provider credentialing and ongoing performance review
- Handling escalations, peer review, and quality assurance
- Coordinating adverse event review and corrective action
ForbiddenRx, as the technology platform, does not direct, influence, or interfere with any clinical decision made by a provider. The Medical Director and treating provider have sole authority over clinical care.
4. Telehealth Network
The Medical Group delivers telehealth services through a vetted clinical infrastructure partner that supplies the licensed-provider network, scheduling, and secure video. All providers in the network operate under the Medical Group’s clinical protocols and credentialing standards described above. The platform is HIPAA-compliant and protected health information is handled under a Business Associate Agreement.
5. Knowing Who Will Treat You
At the start of every visit, your treating provider will introduce themselves by name and credentials. You will see the provider’s name on any prescription issued to you and in your patient record. You may at any time:
- Request the name, credentials, and state license number of your treating provider through the patient portal
- Email support@forbiddenrx.co for provider information
- Decline care from a specific provider and request reassignment
6. Verifying a Provider’s License
Every state medical board publishes a free, public license-lookup tool. Once your treating provider has been disclosed to you, you can independently verify their license, board certification, and any disciplinary history through:
- Your state’s medical board (the Federation of State Medical Boards maintains a directory at fsmb.org)
- The American Medical Association Physician Finder at apps.ama-assn.org/doctorfinder
- The American Board of Medical Specialties at certificationmatters.org
7. State Licensure Coverage
Telehealth in the United States is regulated state-by-state, and your visit must be conducted by a provider licensed in the state where you are physically located at the time of the visit. The Medical Group’s active state coverage is reflected on our State Availability page. If we do not currently have a licensed provider in your state, you can join a waitlist there and we will notify you when coverage opens.
8. Independence of Clinical Judgment
Your provider may, in their sole clinical judgment, decline to prescribe a requested medication, recommend an alternative treatment, refer you to in-person care, or terminate the visit if telehealth is not safe or appropriate for your situation. Payment of any consultation fee does not entitle you to a prescription. The provider’s decision is final and may not be overridden by ForbiddenRx.
9. Complaints and Concerns
If you have a concern about the care you received, email support@forbiddenrx.co and we will route your complaint to the Medical Group’s Medical Director and document the resolution in your record. You also have the right to file a complaint directly with the medical board in the state where you received care, and with the U.S. Department of Health and Human Services Office for Civil Rights for matters involving protected health information.
